doctor switches monthly — set the whole year here in one pass. This is NOT
the "On call" work queue; it is the after-hours doctor. There is no backup
chain: a month with no doctor reaches no one, so an empty month is flagged
until it is filled.
Choose a single practice to set its on-call schedule. On-call coverage is
per-practice; a combined view cannot show one doctor for two practices.
